Ayushman Bharat is a scheme with which you can avail up to 5 lacs worth of free treatment offered by the Government of India. But if the health card is not issued to you due to reasons like your name not being included in the scheme, then here is the way to get your name included in the Ayushman Bharat scheme by yourself.
Add a New Member to the Ayushman Bharat Scheme
To add a member to Ayushman Bharat scheme visit https://cgrms.nha.gov.in/
From the homepage, under the login section solve the captcha security code.
And from the Scheme select the scheme in which you have been already registered, if your name was not listed under PMJAY then select PMJAY from the list, however if you are Building and Other Construction Worker (BOCW) and if you have card then you can select this option too.
Next from the Register Grievance As* select Others as you don’t have the card already.
Enter your mobile number and click on Verify button.
You will need to enter the received OTP in enter the OTP space, solve the below captcha security and click on the Submit button.
On the next page you will be prompted to enter your 12 digits Aadhaar Number, enter your Aadhaar number correctly and click on Validate button.
You will receive OTP on your Aadhaar registered mobile.
Enter the received OTP code and click on the Verify button.
A popup will be displayed “Aadhaar Verified Successfully”.
Click on OK to close the popup.
From the page click on Grievance By, this will expand a form in which you will need to enter applicant details.
As you have already made selection of PMJAY or any scheme which you have selected then those details will be already selected as well as applicant type Others.
Then you will need to select State, District, enter your name, select DOB, select gender, enter email, address, pincode, contact number and additional contact number.
Once filling these details scroll down and under Grievance Details. From the Grievance type select Eligibility related as you wish to apply for new Ayushman Bharat card.
You will get to see a popup wherein you will be asked to upload your documents such as Aadhaar Copy and Ration Card. Make sure the ration card includes your name.
By selecting the applicable option upload the valid documents from your device and click on OK button.
From the Grievance Sub type select Name not in the list and in Grievance Against select SHA.
In the grievance/application description mention your full name and mention that your name has not been included in Ayushman Bharat and request them to include your name and issue you the Ayushman Bharat card ASAP as you belong to below poverty and don’t have fixed source of income or have lower income and this card is very essential for you.
In upload supporting document you may upload your income certificate if you have, or if you have any underlying sickness then you may upload receipt of the treatment which you wish to take.
And in upload audio or video files you may click a video with your disability if you have or if not then you may skip it.
Next click on the Submit button.
A popup will appear with a message “Grievance registered successfully” with the grievance ID which can be used for future reference.
You need to note down this number for future reference purpose or track the status.
On next page your submission status will be displayed as Submitted with all your submitted details.
Now you will need to wait for your application to be reviewed and if your name gets included in the Ayushman Bharat then you will receive SMS on your registered mobile number.
And later on you can download your Ayushman Bharat card online. In between before approval you may also be contacted by officer for additional details.
