Shakti Smart Card Documents Required
To apply for the Shakti Smart Card, the Women Beneficiary will need to furnish the below documents, which are mandatory in order to obtain the card.

From the above POA and POI you need to submit any of the one document to apply for the Shakti Smart Card.
